Talk to a small hungry shop that has been in business for a year or two. Tell what you want to do.

We are a full service shop and I have a small network of windshield repair only businesses. I pay anywhere between $25-50 bucks. The rate just depends on weather it's a cash deal or insurance and the type of vehicle.

I can fax you what we do because it's too wordy to post here.

As far as who collects the money we collect and then pay the fees. We handle all the warranty work as well. Plus we call all of our clients back a week after install just to make sur there are no problems. Our windshield repair people like how we do things.
