Here is is the disclaimer I am putting on the reverse side of my invoices. I am ordering carbonless invoice books.

*Disclaimer:

I authorize Chips-B-Gone Windshield Repair & Headlight Restoration, further known as CBG, to attempt to repair the damage to my windshield or another piece of laminated glass and/or restore the finish to the headlights on my vehicle.
Due to the process of using pressure and vacuum cycles to repair laminated glass and the unstable properties of broken glass, the damaged area may develop additional cracks or existing cracks may increase in length and may become unrepairable. This is also known as the damage has “cracked out”. This problem is very rare and occurs in less than .1% of all windshield repairs performed. In the event the windshield does crack out during the repair, I do not hold CBG, nor its’ technician, liable for the damage.
In the unlikely event that we cannot repair the damage to your satisfaction, on your windshield or headlights, our liability is limited to a refund of any amount charged for the attempted repair and you will be responsible for any deductible incurred for replacement per your insurance policy.
Each repair is different. The damage to the windshield will never completely disappear but will improve the appearance and will prevent the damage from spreading. Headlight restorations will also improve the appearance and visibility. If the restoration is unsuccessful I understand that I will be responsible for the replacement costs of the headlight lenses. I have been instructed as to the expectations; warranties and disclaimers involved with windshield repair /headlight restoration and understand our policies regarding these matters.
