I as well use a 2part form. Ours is more basic, but there is room to write any other necessary info if you need to fax in a copy to the insurance company. With the lack of need to fax all info in, I've opted to make a smaller invoice 8 1/2 x 5 1/2 (The printers put two per page and cut it in half) to get more per order.
