The best UV shield out there is from Liquid Resins. Good size, and lets light (NOT UV) through so its easy to watch the repair.
The crack out. Any chance you forgot to back the injector and feet back out? (as mentioned) Also where was the cup located? Near the edge, near the center, etc. I like it toward the center and away from legs if passable. Pay close attention to breaks on curved glass. ALWAYS watch the break as you tighten the injector and feet. You can sometimes catch a chip that wants to take off. What was the temperature of the glass? Could it have gotten to hot? I also aways crack a window. This prevents pressure when you shut a door. Just a few thoughts.
