Waiver Form, as well as contact

Hello all, I was wondering what type of waiver forms/ form is used when a customer signs to say they let you do the repair. If you have any information on this subject please pass it on:) ...
-As well I am down in Pensacola, Florida and am looking to gain experience in repairing. I am leaving in 2 weeks to return to NJ and am looking to do WSR up their...If anyone is close by or knows of contacts that would be willing to show me a couple of WSR in progress please let me know.
-Well I might as well ask another, on the previous posts their was a mention about a sheet of #'s of insurance companies that accept direct billing. It was under Contracts and billing. If anyone happens to have? Would I be able to obtain it? Thanks Much

PureGeniusbutslow,

I don't recall seeing a list of insurance companies showing who accepts direct billing on here. If there was one not sure if it would cover the state your interested in conducting business in anyway. I personally have never used a separate form signed by the customer when I've direct billed. I simply called their agent and ask them for approval and billing address prior to repair.

Not sure if this helps, maybe others on here have more to add;)

PureGeniusbutslow, If you post an e-mail address, or accept PM, I can send you a copy of what I use. I would say mine is a Payment Authorization/Waiver combination on my invoice.
