Invoice Design/Waiver

Hey everybody...
I am currently redesigning my invoice for WSR and Replacement. Just a few qs for all you experts out there:
I have included a windshield design including type of repair and replacement details ...any new designs out there?
Does anyone have any info relating to the waiver? I have included that there is a workmanship lifetime guarantee... that the repair/replacement will be covered as long as it is owned by the same person who got the repair etc .. however I have noted that in the US a waiver is included to incorporate that any repair that goes into a crack or something to that effect, is not covered and also that if this 'problem' arises that a refund will not be given or that this happens in 0.01% or it is extremely rare. Wouldnt this scare customers away as in Canada everyone bands about 'lifetime guarantee' and I have rarely seen this included in an invoice here. What has your experience been like? What should be included?
