The only disclaimer I use is if the insurance company fails to pay, the customer is responsible for the payment. Other than that, it's just the opposite of a disclaimer... My invoice says if your rapair fails for any reason for the life of the windshield (gets worse, cracks from that spot, etc), we will fix it again or apply the cost of the repair to the replacement, guarenteed.

My disclaimer contains what JAC says and also reminds the customer that they glass was broken when I got there. Should it crack out when I'm working on it, they will have to buy a new piece of glass. I've only presented it once in 10 years because I have been successful at building a good relationship with my customers up front. I still think it's a good idea to have it handy, just in case.

all star, if you want the text, let me know with a private message and I'll email it to you.
