Getting Contracted with Insurance companies

I have gone around in circles with SGC, HSG, and LYNX and trying to become registered so that i can run claims with them. Can anyone explain the process? I have submitted the paperwork they asked for, but they gave no definite instructions on how to file a claim, how to get a shop number, or even who to send the claims to... I tried calling SGC, but all they have is an e-mail for questions, to which they have never replied. I would love some direction in this area.
