Repair Forms

Could anyone point me in the right direction to get a good form for windshield repair. I was thinking something that has a picture of a windshield so I could mark where the damage was. Maybe even a two part form so I could give the customer one and keep one for future use, especially for fleet work. I could then keep it and always have record of where damage was repaired on each vehicle.

One of the things I did before customizing any repair forms was go to staples, office max, etc, and bought gereal repair forms, they actually work really well until you establish your customized form.

I just designed my own invoice on Word or Microsoft Works and went to Kinko's and had them make me up a few hundred carbonless two-part forms from that original. They actually look very professional. I keep the top (white copy) and the customer gets the yellow copy.

There are some specific things I have to have on my invoices for insurance billing that is required by the insurance companies, such as...

And finally "Damage Location Codes"

Damage Location Codes is just a way for me to identify the location on the windshield where the chip was located.

D=Driver Side, C=Center, P=Passengerside, U=Upper, M=Middle, L=Lower

I simply circle one or more of the letters to help identify the location.
A picture of a windshield would be nice, but I could not find one I liked to use on my invoice, so I dreamed up the Location Codes.

When I write an invoice in the field, I write my own invoice number. Invoice #__________ and I just fill in the date.
My invoice numbers look like this... 020706-1 This invoice number tells me that it was the first job I did on Feb 7th, 2006.
