I'm looking to put together a signoff sheet for my customers prior to starting the repair. Basically educating them on what to expect with a repair and the risk involved in performing a repair. Its not so much for liability but rather for education purposes. Do any of you have something like this if so would you like to share some ideas. Pm would be ok.

I prefer the spoken word rather than the written but your right in educating the customer before starting. I check their expectations for the completed job and run through the pros and cons, no one yet has changed their minds.
Is a customer always right? - No, but they are always the customer.
